Job summary

Trinity Health PACE is seeking a Registered Nurse for a full-time role at the PACE St Joseph location. The RN will provide skilled nursing care, develop care plans, and coordinate with the interdisciplinary team to optimize participant health and well-being.

Minimum qualifications include an active RN license (BSN preferred) and CPR certification, with at least 2 years of geriatric care experience. Day-1 benefits, DailyPay, PTO, and tuition reimbursement are offered.

Qualifications

• Graduation from an accredited nursing program with an active, unencumbered RN license.
• Experience with geriatric population.
• CPR certification required.

Responsibilities

• Provide skilled nursing care, including administering treatments and medications as prescribed.
• Conduct assessments, develop care plans, and coordinate with IDT to monitor participant health.
• Educate participants and families on self-care and health management.
• Ensure adherence to nursing standards and regulatory requirements.
• Serve on committees and participate in QAPI activities.
• Maintain accurate documentation of participant care.
