Job summary

A-Team Solutions is hiring a Nurse Case Manager (RN) in Phoenix, AZ to support AZARNG Office of the State Surgeon. The role coordinates medical readiness activities, liaises with Soldiers, military providers, and unit leadership to ensure deployable status.

Responsibilities include managing medical readiness cases, arranging treatment plans, and maintaining HIPAA-compliant records across systems such as MEDPROS and eCase, with occasional clinical events and drills.

Qualifications

• Active, unrestricted RN license.
• BSN degree required.
• AHA BLS certification current.
• Minimum 2 years nurse case management or related nursing experience.
• Strong knowledge of medical documentation and healthcare regulations.
• Ability to obtain government credentials and installation access.

Responsibilities

• Manage medical and dental readiness cases for Soldiers.
• Conduct initial and follow-up case management interviews about deployment-limiting conditions.
• Coordinate treatment plans, referrals, and follow-up care.
• Maintain Soldier medical readiness information in databases (eCase, MEDPROS, MEDCHART, MODS, eProfile, HRR, DENCLASS).
• Review medical documentation to support readiness actions (MEBs/PEBs).
• Assist with SRP events and PHAs.
• Ensure medical records meet Army regulations and HIPAA requirements.
• Educate Soldiers on medical, VA, TRICARE, and community resources.
• Prepare provider review packets and readiness reports.
• Provide weekly updates on medically non-deployable cases.
• Coordinate with military treatment facilities, VA, and other providers.
• Participate in readiness meetings and provide status reports.
• Support deployment readiness initiatives.
