Job summary

Luke AFB in Glendale, AZ seeks a Registered Nurse – Case Management to oversee assessment, planning, implementation, and coordination of care for service members and families. The role emphasizes discharge planning, resource optimization, and patient advocacy within DoD guidelines.

The RN will collaborate across disciplines, ensure timely services, and document outcomes while maintaining high standards of care and compliance with CMSA/URAC guidelines.

Qualifications

• Associate or BSN in Nursing from ACEN/CCNE accredited program.
• 3+ years' RN experience within last 4 years, full-time in clinical case management.
• Current RN license; ability to obtain DoD background clearance.

Responsibilities

• Participate in all phases of the Case Management Program and meet CM standards of care.
• Provide nursing expertise across assessment, planning, implementation, coordination, and monitoring.
• Develop and implement CM strategies across inpatient, outpatient, onsite and telephonic care.
• Create tools to support case management, such as patient identification/assessment guides and CM software.
• Coordinate with community agencies and document patient care data.
• Develop treatment plans including preventive, therapeutic, psychosocial, and clinical interventions.
• Educate patients and families to ensure understanding and adherence to care plans.
