Nurse Case Manager II - Clinical - Hybrid

Position Type: 4‑month contract
Location: Hybrid (field and remote, county locations in IL – can live anywhere in IL; travel required within IL)
Schedule: M‑F 8am‑5pm Central
Pay Range: $30.00–$36.00 per hour

Position Summary

Clinical Case Manager responsible for assessment, planning, facilitation and advocacy to meet an individual’s benefit plan and/or health needs. Utilizes collaborative processes, clinical tools, and available resources to promote optimal, cost‑effective outcomes.

Responsibilities

• Conduct comprehensive assessments of referred members’ needs and eligibility using clinical tools and data review.
• Determine approach to case resolution, evaluating member benefits and available internal and external programs/services.
• Apply and interpret applicable criteria, guidelines, and standardized case management plans while ensuring accurate benefit administration.
• Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies.
• Collaborate virtually with teams, using appropriate tools, and maintain independent critical thinking.
• Ensure regular communication and coordination with internal providers, external agencies, and members.

Qualifications

• Minimum 3–5 years of clinical practical experience (preferred).
• Minimum 2–3 years of Care Management, discharge planning, and/or home health care coordination (preferred).
• Strong analytical and problem‑solving skills.
• Effective communication, organizational, and interpersonal skills.
• Independent work ability and self‑direction.
• Proficiency with standard corporate software (MS Word, Excel, Outlook, PowerPoint) and multiple proprietary applications.

Required Licenses

Unrestricted State Licensure in IL as an LCPC, LCSW, or RN (requires unrestricted active license).

Education

RN, LCPC, or LCSW with current unrestricted state licensure.

Desired Certifications

Case Management Certification (CCM) preferred.
