Our Client, a Retail Pharmacy company, is looking for a Nurse Case Manager II for their Southside of Chicago, IL/ Suburban Cook, IL (Chicago Heights, South Chicago Heights, Lynwood, Ford Heights, Sauk Village, Matteson Olympia Fields, Park Forest, Richton Park) location.
Responsibilities:
- The Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
- Conducts comprehensive assessments of referred members' needs/eligibility using clinical tools and information/data review, and determines approach to case resolution by evaluating member's benefit plan and available programs/services.
- Applies and interprets applicable criteria, guidelines, standardized case management plans, policies, procedures, and regulatory standards to assess benefits and member needs, ensuring proper administration of benefits.
- Utilizes case management and quality management processes in compliance with regulatory and accreditation standards, company policies, and procedures.
Requirements:
- RN, LCSW, or LCPC with an unrestricted active license is required.
- Experience with case management and IL waiver services is preferred.
- Minimum 3-5 years of clinical practical experience preferred.
- Minimum 2-3 years of care management, discharge planning, or home health care coordination experience preferred.
- Ability to work independently in a remote setting, using tools to collaborate and connect virtually.
- Excellent analytical, problem-solving, organizational, and interpersonal skills.
- Effective communication skills and proficiency in navigating multiple systems and software, including MS Word, Excel, Outlook, PowerPoint, and proprietary applications.
