Job description

The Remote Patient Monitoring (RPM) Home Telehealth (HT) Nurse Case Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of patient care. HT is a program that coordinates care using health informatics, disease management, and home telehealth technologies to facilitate access to care and improve the health of Veterans, with the goal of providing care in the right place at the right time.

This is not a remote position.

Qualifications
Basic Requirements:
  • English Language Proficiency: In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE at the time the program was completed. Alternatively, individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have the opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon graduation. Foreign graduates must possess a current, full, active, and unrestricted registration, with degree equivalency validated if higher than an associate degree.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the U.S., or the District of Columbia. Graduate Nurse Technicians (GNT) may be appointed temporarily while pursuing licensure.

NOTE: Grandfathering provision applies to current VHA employees performing duties as of 1/29/2024.

Grade Determinations

Summary of grade levels:

  • Nurse I, Level I: Basic care, ADN or Diploma.
  • Nurse I, Level II: Biopsychosocial integration, ADN or Diploma + 1 year, or BSN.
  • Nurse I, Level III: Proficiency, ADN or Diploma + 2 years, or BSN + 1 year, or MSN.
  • Nurse II: Leadership, BSN + 2 years, or MSN + 1 year, or related degrees with experience.
  • Nurse III: Complex care leadership, MSN + 2 years, or related degrees with experience.

*Note: Foreign education programs/degrees are not creditable as related degrees.

Preferred Experience
  • Experience in Case Management.
  • Previous experience in home monitoring.
  • Experience with individuals with complex medical needs.
  • Experience in Acute or Primary Care.
