A leading federal agency is seeking a Remote Patient Monitoring (RPM) Home Telehealth Nurse Case Manager responsible for coordinating care using technology to improve Veterans' health outcomes. Qualified nursing professionals with experience in case management and home monitoring are encouraged to apply. This role requires strong leadership and patient care management skills.
The Remote Patient Monitoring (RPM) Home Telehealth (HT) Nurse Case Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of patient care. HT is a program that coordinates care using health informatics, disease management, and home telehealth technologies to facilitate access to care and improve the health of Veterans, with the goal of providing care in the right place at the right time.
This is not a remote position.
