SFM Mutual Insurance Company is seeking a Nurse Case Manager in Minnesota. You will manage workers’ compensation claims primarily by phone, providing medical guidance, coordinating care, and promoting timely return-to-work for injured workers.

The role emphasizes clinical expertise, adherence to state laws, and collaboration with the claims team to deliver high-quality service and cost-effective outcomes.

Qualifications

• RN licensure in the hiring state; multi-state licensure preferred.
• 2 years of case management or related health experience.
• CCM/CDMS/CRRN certifications preferred.
• Five years of clinical experience preferred.

Responsibilities

• Provide telephonic medical case management for workers’ compensation claims.
• Meet injured workers and policyholders in person or virtually to support outcomes.
• Coordinate return-to-work plans using guidelines and medical data.
• Educate on treatment parameters and disability duration for recovery.
• Offer clinical guidance on treatment timing, cost containment, and returns-to-work.
