Job summary

The NCM works with care teams, navigates barriers to care, and may involve outreach to families and community partners to ensure timely follow-up and reduced hospital readmissions.

Qualifications

• Active RN license and ability to practice in Arizona.
• Experience in outpatient case management and care coordination.
• Strong communication and patient education skills.

Responsibilities

• Provide outpatient case management and self-management education for patients with chronic disease.
• Coordinate care across primary and specialty services and assist with transportation and scheduling.
• Educate patients and families on services, risks, benefits, and alternatives; maintain accurate records.
