Job summary

Qualifications

• RN license and clinical experience in case management.
• Knowledge of chronic disease management and care coordination.
• Ability to educate patients and families on services and self-management.
• Familiarity with community resources and HIPAA compliance.

Responsibilities

• Works with the primary care team to help patients access specialty medical care.
• Coordinate care plans for patients with complex health needs and chronic disease.
• Educate patients and families about services, benefits, risks, and alternatives.
• Arrange transportation and scheduling for specialty appointments and tests.
• Perform chartReviews and document education and outcomes in the medical record.
• Maintain confidentiality and HIPAA compliance across all activities.
