Job summary

TRIUNE Health Group Inc. is seeking a Nurse Case Manager in Marshfield, WI to coordinate resources and develop cost-effective, holistic treatment plans for catastrophically or chronically ill patients.

You will work with patients, physicians, employers, and referral sources to ensure quality care and timely return to work when appropriate. Qualified candidates have an RN license in Wisconsin, 3+ years of clinical experience, and CCM is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of operation.
• Three or more years of recent clinical experience, preferably in trauma, psychology, emergency, orthopedics, rehabilitation, occupational health, and neurology.
• CCM preferred.

Responsibilities

• Provide medical case management coordinating with patient, physicians, and employers.
• Utilize case management steps: assessment, planning, implementation, evaluation, and outcome.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
• Facilitate care and coordinate durable medical equipment and home health services.
• Communicate medical information clearly to patients and families.
• Stay current with health care laws including HIPAA, FMLA, and workers' comp regulations.
• Prepare timely reports using computer, phone, and scanning equipment.
