Job summary

UF Health in Leesburg, FL seeks a Case Manager to assess patient and family scenarios, develop care plans, and coordinate along the continuum of care. The role requires effective communication with physicians and staff, minimum 2 years in case management preferred, and Florida RN license.

Qualifications
