Job summary

Paradigm is seeking a full-time Field Nurse Case Manager to cover Raleigh and nearby communities, including Goldsboro. This field/home-based role involves travel to doctors’ offices, hospitals, and other sites, with up to 2 hours one-way per appointment.

The role focuses on medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation while coordinating with insurers, providers, attorneys, employers, and employees.

Qualifications

• Current, unencumbered RN license in NC.
• Rehabilitation Professional certification required within 60 days of hire.
• Experience in workers' comp case management preferred.
• Certifications CCM, CRRN, CDMS, CRC preferred; REQUIRED within two years of hire.
• Strong organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, interpersonal).
• Ability to operate autonomously with minimal oversight.
• Valid Driver’s License with good driving history.

Responsibilities

• Provide medical case management for work-related injuries; assess, plan, coordinate, implement and evaluate care.
• Collaborate with insurance carriers, medical care providers, attorneys, employers, and employees; report results to employer and carriers.
• Conduct field visits to doctors' offices, hospitals, and other locations; travel up to 2 hours one-way (4 hours round trip).
• Cover Raleigh and nearby communities, including Goldsboro.

We are seeking a full-time Field Nurse Case Manager to cover the Raleigh and nearby communities, including Goldsboro.

A Field Case Manager role is a field / home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). These appointments would be to doctors’ offices, hospitals, and various other locations. There may be multiple appointments in 1 day.

This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.

Qualifications:

• Current, unencumbered compact Registered Nurse (RN) license in NC.
• Registered as a Rehabilitation Professional with the North Carolina Industrial Commission required within 60 days of hire.
• Experience in case management of workers comp cases, preferred
• Recommended certifications (CCM, CRRN, CDMS, CRC) preferred. REQUIRED within two years of the date of hire
• Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
• Ability to operate autonomously with minimal oversight
• Skilled at patient education
• Valid Driver’s License, and good driving history
