We are seeking a Field Nurse Case Manager to cover the Northern Bay Area, CA, and surrounding areas. This is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and other locations. The individual is responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager collaborates with insurance carriers, medical providers, attorneys, employers, and employees, monitoring the injured worker’s progress and reporting results to the employer and insurance carriers.
