Job summary

Merakey is seeking a Registered Nurse Case Manager in Warminster, PA to provide direct nursing care and coordinate with a multidisciplinary team across group homes in Phila, Bucks, and Montgomery Counties.

The role offers a $43/hour base with an eligible enhanced pay of $2/hour, in-office duties with afternoon travel, and a M–F 9:00 AM–5:00 PM schedule.

Qualifications

• Valid RN licensure in PA.
• Graduated from an accredited RN program.
• Prefer 1+ year of RN experience.

Responsibilities

• Administer medications and treatments, including tube feedings and tracheostomy care.
• Perform nursing assessments to identify health issues.
• Collaborate with healthcare professionals to coordinate care.
• Provide health education to residents and families.
• Conduct staff in-service training to ensure protocol compliance.
• Communicate health issues with the team and participate in meetings.
• Complete quarterly assessments with accurate documentation.
• Fulfill charge nurse duties when assigned and respond to emergencies.
• Perform other duties to support program operations.

Job description

We are seeking a Registered Nurse Case Manager to join our team in Warminster, PA.

Earn: $43 / Hour + Enhanced Pay Eligible (Additional $2/hour)

Location: In office plus afternoon travel to one or more of our group homes in Phila, Bucks and Montgomery Counties.

Shift: M - F 9:00 AM - 5:00 PM

As an RN at Merakey, you'll play a vital role in providing direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care.

Essential Job Functions:

• Administer medications and treatments, including tube feedings and tracheostomy care, with precision and compassion.
• Perform nursing assessments on residents as needed to identify and address health issues promptly.
• Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
• Provide health education to residents and their families, including implementing self‑medication programs as directed by the nurse manager.
• Conduct in‑service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
• Communicate health issues with the interdisciplinary team, actively participating in staff meetings and monthly reviews with the QIDP.
• Complete quarterly assessments in a timely manner, maintaining accurate documentation.
• Fulfill duties as a charge nurse when assigned, carrying an emergency contact telephone, and responding as required.
• Perform other duties as assigned, contributing to the smooth operation of our program.

The ideal candidate will possess the following qualifications:

• Graduation from an accredited Registered Nurse program.
• Current license as an RN in the Commonwealth of PA.
• Preferably, at least one year of experience as an RN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Ability to perform professional nursing duties including assessment, treatment, and medication dispensing.
• Physical capability to exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.
