Merakey is seeking a Registered Nurse Case Manager for our Warminster program in PA. The role involves in-office nursing care and afternoon travel to group homes across Bucks, Philadelphia, and Montgomery Counties. Schedule is Monday through Friday, 9:00 AM to 5:00 PM.

Earn $43 per hour with an enhanced pay eligible addition of $2 per hour. Responsibilities include clinical assessments, medication administration, and coordinating care with a multidisciplinary team.

Qualifications

• Graduation from an accredited Registered Nurse program.
• Current PA RN license.
• Preferably 1 year RN experience.
• Interest in individuals with disabilities.
• Valid PA driver license and reliable transportation.

We are seeking a Registered Nurse Case Manager to join our team at our program in Warminster, PA.

Shift: M - F 9:00 AM - 5:00 PM

As an RN at Merakey, you'll play a vital role in providing direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care.

Essential Job Functions:

• Administer medications and treatments, including tube feedings and tracheostomy care, with precision and compassion.
• Perform nursing assessments on residents as needed to identify and address health issues promptly.
• Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
• Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
• Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
• Communicate health issues with the interdisciplinary team, actively participating in staff meetings and monthly reviews with the QIDP.
• Complete quarterly assessments in a timely manner, maintaining accurate documentation.
• Fulfill duties as a charge nurse when assigned, carrying an emergency contact telephone, and responding as required.
• Perform other duties as assigned, contributing to the smooth operation of our program.

The ideal candidate will possess the following qualifications:

• Graduation from an accredited Registered Nurse program.
• Current license as an RN in the Commonwealth of PA.
• Preferably, at least one year of experience as an RN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Ability to perform professional nursing duties including assessment, treatment, and medication dispensing.
• Physical capability to exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.
