Job summary

Paradigm is seeking an experienced Field Nurse Case Manager to support the Atlanta, GA area. This field/home-based role involves travel to physicians' offices, hospitals, and other care locations.

You will manage medical case work for work-related injuries, including assessment, planning, coordination, and evaluation of care, while collaborating with workers, carriers, providers, and attorneys. Excellent communication and independent work are essential.

Qualifications

• Current Registered Nurse (RN) license in Georgia.
• Nationally recognized case management certification: CCM, CRRN, COHN, or CDMS.
• Valid driver’s license with good driving history.

Responsibilities

• Conduct comprehensive assessment, planning, coordination, implementation, and evaluation of care.
• Coordinate with injured workers, insurance carriers, medical providers, employers, and attorneys.
• Monitor clinical progress and outcomes and report to stakeholders; manage complex and catastrophic cases.

• Catastrophic managed cases, which may pay $250 + per case
• Premium hourly pay for qualifying catastrophic cases (additional $8-$10 per hour)
• Productivity and premium bonuses, including a 5% premium bonus paid over productivity requirements
• Mileage reimbursement (federal rate), cell phone, and internet reimbursement

About the Role - This is a field / home-based field position with travel (up to 2 hours one-way) to physicians' offices, hospitals, and other care locations.
