Job summary

Paradigm is seeking a Field Nurse Case Manager to cover Atlanta area including Athens, Gainesville, Lawrenceville, Douglasville and McDonough. This hybrid role requires location in the Atlanta area due to regular local travel (80% travel) and GA licensure.

The nurse will manage medical cases for work-related injuries with input from carriers, providers, attorneys, employers, and employees. Responsibilities include assessment, planning, coordination, implementation and evaluation of cases.

Qualifications

• Current Registered Nurse (RN) license in GA.
• CCM, CRRN, COHN, or CDMS required.
• GA Rehabilitation Supplier preferred, required within one week of start date.
• Experience in workers compensation case management, preferred.
• Skills: organization and timeline adherence, PC/technology, communication.
• Valid Driver’s License and good driving history.

Responsibilities

• Medical case management of work-related injuries: assessment, planning, coordination, implementation, evaluation.
• Collaborates with insurance carriers, medical care providers, attorneys, employers, and employees.
• Willing to learn and handle catastrophic cases.

We are seeking a Field Nurse Case Manager to cover Atlanta to include but not limited to Athens, Gainesville, Lawrenceville, Douglasville, McDonough.

A Field Nurse Case Manager is a hybrid position. The candidate must be located in the Atlanta area due to regular local travel (80% of the time) for in-person patient appointments. Candidate must be willing to travel and be licensed in GA.

This individual is responsible for the medical case management of work‑related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Candidates must be willing to learn and take catastrophic cases.
