Merakey seeks an LPN/LPN- or RN-qualified Case Manager to deliver community-based services across Lehigh & Northampton Counties. You will support adults with mental health and medical needs through in-home visits, care coordination, and advocacy, collaborating with interdisciplinary teams.

The role emphasizes person-centered care, relationship-building, and flexible scheduling with an on-call rotation. Compensation includes an hourly wage with enhanced pay option.

Qualifications

• Must hold RN or LPN licensure (or RN with bachelor’s degree and social science credits).
• Minimum 2 years of experience in a mental health setting.

Responsabilités

• Coordinate and implement services per person-centered service plan.
• Manage and coordinate consumers’ overall medical care including physician orders and meds.
• Provide health education to explain conditions, treatment plans, and wellness goals; advocate at medical appointments.
• Deliver case management, supportive counseling, and skills training in a community-based setting.
• Participate in an on-call rotation to respond to urgent consumer needs.

Merakey is seeking a Nurse (LPN) Case Manager to provide community-based services throughout Lehigh and Northampton Counties. This role focuses on supporting adults with both mental health and medical diagnoses through in-home visits, care coordination, health advocacy, and collaboration with interdisciplinary providers. The ideal candidate is passionate about delivering person-centered care and building meaningful relationships that help individuals thrive in their communities.

The LPN Case Manager will be responsible for:

• Coordinating and implementing services in accordance with each individual's person-centered service plan.
• Managing and coordinating consumers' overall medical care, including monitoring and following through on physician orders and medication recommendations.
• Providing health education to promote understanding of medical conditions, treatment plans, and wellness goals, while advocating for consumers during medical appointments.
• Delivering case management, supportive counseling, and skills training services using a flexible, community-based approach.
• Participating in an on-call rotation to provide support and respond to urgent consumer needs as required.

Earn: $31.98 per hour.

*An Additional $2 per hour is available with the selection of our Enhanced Pay Option*

Requirements

The ideal candidate will possess the following qualifications:

• Must be a Registered Nurse, Licensed Practical Nurse or bachelor's degree in nursing with a minimum of 12 credit hours in social science.
• a minimum of 2 years of experience in a mental health setting.
• Valid PA Driver's License and vehicle for travel to clients-Mileage reimbursement provided
• Open to working out in the community in Lehigh and Northampton counties Monday-Friday, 8am-4:30pm.
