Job summary

UVM Health Network seeks a Manager of Case Management to lead the team at Central Vermont Medical Center in Berlin, VT. The role oversees complex care coordination, discharge planning, and throughput, guiding a high‑performing team focused on quality, safety, and efficient patient flow.

Responsibilities include resource management, budgeting for a $2–3M scope, and using data to drive improvements while ensuring regulatory compliance and interdisciplinary collaboration across the health system.

Qualifications

• Associate Degree in Nursing with active Vermont State RN license required.
• CCM certification required within the timeframe set by leadership upon hire.
• 3+ years clinical setting experience; 5+ years leadership experience preferred.

Responsibilities

• Provide leadership, oversight, and accountability for Case Management program performance.
• Lead a team responsible for complex care coordination across inpatient units and the Emergency Department.
• Ensure resources, efficiency, budget management, and regulatory/compliance alignment.
• Mentor staff, escalate issues, and promote quality, patient safety, and service excellence.
• Drive process improvements and data-informed decision making to advance care management.
