Health Wealth Safe, Inc. seeks a patient‑centered care coordinator to guide patients and families across the care continuum.

You will assess needs, coordinate services, and support transitions between home health, clinics, and community resources. This role includes educating families for understanding and adherence, collaborating with an interdisciplinary team, and supervising LPNs and home health aides to ensure quality and safety.

Formación

• Proven ability to assess patient needs and develop care plans.
• Strong communication and patient/family education skills.
• Demonstrated care coordination and transition support.
• Leadership and supervision of licensed and ancillary staff.

Responsabilidades

• Assess patient needs and develop individualized care plans.
• Coordinate and manage care across settings, including transitions.
• Educate patients and families to support understanding and adherence.
• Supervise LPNs and home health aides to ensure quality and safety.
• Collaborate with interdisciplinary care teams in home health and community settings.
• Support outpatient care management with direct patient involvement.
