Job summary

Martha’s Hands LLC is expanding its Nurse Care Manager team for Private Pay Home Care Clients in the St. Louis area. You will work within a 12-person Care Supervisory Team to guide clients and families, train caregivers, and coordinate with home health, hospice, and other professionals.

Private pay only—no Medicare or insurance paperwork. Responsibilities include assessments, service-plan development, care coordination, caregiver orientation, and direct nursing tasks as needed, including wound

Qualifications

• 3+ years nursing experience required.
• Strong clinical assessment skills to develop customized service plans.
• Excellent interpersonal communication to coordinate with families and professionals.
• Ability to train and supervise caregivers in home care settings.

Responsibilities

• Guide clients and families through care needs and develop service plans.
• Coordinate care with Home Health, Hospice and other professionals.
• Orientate and train caregivers to provide high-quality care.
• Provide direct nursing tasks and medication management as needed.
• Balance office coordination with mobile client visits using technology.
