Job summary

Martha’s Hands LLC is expanding its Nurse Care Manager team for Private Pay Home Care Clients in the St. Louis area. You will work within a 12-person Care Supervisory Team to guide clients and families, train caregivers, and coordinate with home health, hospice, and other professionals.

Private pay only—no Medicare or insurance paperwork. Responsibilities include assessments, service-plan development, care coordination, caregiver orientation, and direct nursing tasks as needed, including wound

Qualifications

• 3+ years nursing experience required.
• Strong clinical assessment skills to develop customized service plans.
• Excellent interpersonal communication to coordinate with families and professionals.
• Ability to train and supervise caregivers in home care settings.

Responsibilities

Martha's Hands Nurse Care Managers:

Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:

• Guide a client and their family through their care needs.
• Support/Train our Caregivers in providing direct care to our clients in their homes.
• Partner with home health, hospice and other care professionals to manage their care.

Our Nurse Care Managers are compassionate, health care professionals who:

• Conduct Assessments with clients and their families in order to develop a customized Service Plan.
• Coordinate care with other health care professionals including: Home Health, Hospice, Primary Care, etc.
• Orientate and train our Caregivers to provide excellent care.
• Provide clinical oversight of our clients and support to our caregivers.
• Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.
• Assist families with more detailed Care Management Services as needed.
• Balance both an office and mobile work environment, utilizing technology to enhance care for our clients and equip our Nurse Care Managers to be more

Requirements
