Job summary

Martha’s Hands is expanding our team of Nurse Care Managers for Private Pay Home Care Clients in the St. Louis area. Join a 12-person Care Supervisory Team to guide clients and families through care needs, train caregivers, and partner with home health and hospice professionals to manage care.

This role balances office and mobile work, ensuring quality care with clinical oversight and direct nursing tasks as needed.

Qualifications

• 3+ years nursing experience in private duty, home health, hospice, geriatric nursing or Med Surg.
• Strong professional, interpersonal communication skills.
• Ability to develop customized service plans from assessments.
• Education and training abilities to enhance caregiver success.

Responsibilities

• Conduct assessments with clients and families to develop customized service plans.
• Coordinate care with other health professionals including Home Health and Hospice.
• Orientate and train caregivers to provide excellent care.
• Provide clinical oversight and support to clients and caregivers.
• Provide direct care including skilled nursing tasks and ADL assistance.
• Balance office and mobile work to enhance care using technology.
• Supervise care team to ensure quality care and motivate caregivers.

Martha's Hands Nurse Care Managers:

Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:

• Guide a client and their family through their care needs.
• Support/Train our Caregivers in providing direct care to our clients in their homes.
• Partner with home health, hospice and other care professionals to manage their care.

Private Pay Clients Only - No Medicare or Insurance Documentation!

Our Nurse Care Managers are compassionate, health care professionals who:

• Conduct Assessments with clients and their families in order to develop a customized Service Plan.
• Coordinate care with other health care professionals including: Home Health, Hospice, Primary Care, etc.
• Orientate and train our Caregivers to provide excellent care.
• Provide clinical oversight of our clients and support to our caregivers.
• Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.
• Assist families with more detailed Care Management Services as needed.
• Balance both an office and mobile work environment, utilizing technology to enhance care for our clients and equip our Nurse Care Managers to be more efficient and effective.
• Supervise care team to ensure quality care expectations are being met while inspiring and encouraging our Caregivers.

Requirements

• 3+ years nursing experience. Ideal experience in Private Duty, Home Health, Hospice, Geriatric Nursing or Med Surg.
