Job summary

Martha’s Hands Home Care & Wellness Services is expanding its Nurse Care Manager team to support Private Pay Home Care Clients in the St. Louis area. You will guide clients and families, coordinate with health professionals, and train caregivers to deliver high‑quality in-home care.

The role balances office and mobile work, performing assessments, care planning and direct nursing tasks as needed. Private pay focus, flexible scheduling and team collaboration are emphasized.

Qualifications

• 3+ years nursing experience.
• Team approach to care by coordinating with home health, hospice and other professionals.
• Strong clinical assessment skills to develop customized service plans.
• Educational and training abilities to enhance caregiver success.

Martha's Hands Nurse Care Managers

Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:

• Guide a client and their family through their care needs.
• Support/Train our Caregivers in providing direct care to our clients in their homes.
• Partner with home health, hospice and other care professionals to manage their care.

Our Nurse Care Managers are compassionate, health care professionals who:

• Conduct Assessments with clients and their families in order to develop a customized Service Plan.
• Coordinate care with other health care professionals including: Home Health, Hospice, Primary Care, etc.
• Orientate and train our Caregivers to provide excellent care.
• Provide clinical oversight of our clients and support to our caregivers.
• Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.
• Assist families with more detailed Care Management Services as needed.
• Balance both an office and mobile work environment, utilizing technology to enhance care for our clients and equip our Nurse Care Managers to be more

Requirements

• 3+ years nursing experience. Ideal experience in Private Duty, Home Health, Hospice, Geriatric Nursing or Med Surg.
• Team Approach to care by utilizing strong professional, interpersonal communication skills.
• Strong clinical assessment skills to develop a customized service plan that directs the care for our clients.
