Job summary

ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions. You will work with the care team to follow patients from clinics into acute/post-acute settings and home environments.

Qualifications

• BSN or RN with bachelor’s degree required or preferred.
• Active RN license in the state of employment is required.
• Minimum 2 years of clinical work experience.
• Desirable: 1 year of utilization review, home health, discharge planning.
• Desirable: 1 year of acute or ambulatory case management experience.
• Certification as Case Manager (CCM/CMCN) preferred or willingness to obtain.

Responsibilities

• Establish trusting relationships with patients and families/caregivers.
• Collaborate with care team to achieve patient goals.
• Coordinate and evaluate management across acute and chronic conditions.
• Develop comprehensive individualized care plans and execute them.
• Perform case management following CMSA standards.
• Provide patient education and care coordination to achieve health goals.
• Use Motivational Interviewing to engage patients.
• Communicate with hospital case managers and care providers to facilitate safe discharge and transitions of care.
