Job summary

Northwestern Medical Center in Vermont seeks a Nurse Care Manager in Case Management. This per-diem role runs primarily 8:00 a.m.–4:30 p.m., with roughly one weekend per month.

Qualifications

• RN license in Vermont (or eligible) with current licensure.
• BSN or MSN strongly preferred.
• Three years in hospital and/or clinic setting preferred.
• Certified Case Manager preferred.

Responsibilities

• Conduct individualized concurrent clinical evaluations of patients and review medical records.
• Develop personalized action plans, provide education and referrals for post-acute care.
• Monitor patient progress and modify care plans; coordinate discharge planning and transitions.
• Collaborate with physicians, nurses and other healthcare professionals to optimize patient outcomes.
