Job summary

Northwestern Medical Center is seeking a Social Services Care Manager in Case Management. The role is per-diem with typical hours 8:00 a.m. - 4:30 p.m., generally one weekend per month. You will evaluate patients, plan post-acute care, coordinate with care teams, and support transitions to community-based care.

Qualifications include a Master's in Social Work, Counseling, or Psychology, LMSW preferred, and at least three years in hospital or clinic case management. BLS certification is required.

Qualifications

• LMSW strongly preferred.
• Three years hospital/clinic case management experience preferred.
• Certified Case Manager preferred.
• BLS certification required.

Responsibilities

• Conduct individualized clinical evaluations, review records and develop action plans.
• Coordinate post-acute care referrals and monitor progress with the care team.
• Assist with discharge planning and transitions to community or long-term care.
• Communicate with health insurers, patients and families to support care needs.
• Maintain accurate patient records and uphold patient confidentiality.
