Seaboard Foods in Guymon, OK is seeking a Registered Nurse Care Manager to oversee the medical management program for all plant employees. This office-based role focuses on developing individualized plans of care, providing education, guidance and support for employees with health conditions, and coordinating care with medical providers.

We have an excitingNurse Care Manageropportunityin ourplant operationsin Guymon,OK.The Registered Nurse Care Manager is an office-based medical and telephone medical care management position that is responsible for managing the Seaboard Foods medical management program with an emphasis on developing patients’ plans of care, providing education, guidance, and support for individuals with underlying health conditions for all employees at Seaboard Foods processing plant. This individualis responsible forearly intervention and assessment,appropriate testing, medical care coordination, recordability review, and communication withappropriate governmentagencies and internal stakeholders.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Manage a medical management system toassistemployees with underlying health conditions.

• Conduct initial and ongoing patient assessments, including medical history and record reviews, and develop individualized plans of care in collaboration with the employee and their primary medical provider.Act asliaisonbetween the employee and medical providers to ensure all employees receivetimelyaccess to quality medical care while promotingoptimalclinical outcomes.

• Facilitate andmonitorpatient outreach and education to improve employees’ overall health and care while the medical treatment and virus management progress for all employees while working as an intermediary between medical care providers, management, and employees.

• Proactivelyidentifytrends, conditions, and workplace factors that may contribute to employee health concerns and developappropriate preventivehealth initiatives.

• Promote early identification and intervention for potential medical issues to help minimize health-related disruptions and improve employee well-being.

• Develop and implement employee health programs focused on wellness, health education, disease prevention, andmaintaininga healthy workforce.

• Monitors patient progress against the plan of care goals, emphasizing patient care needs during transitions and health changes.

• Assistemployees in explaining and reinforcingphysicians’instructions and answers to other questions the employees may haveregardingtreatment plans.

• Educate employeesregardinghealth promotion, disease prevention, ergonomics,appropriate useof medical services, and early reporting of health concerns.

• Maintainaccurateand confidential employee medical recordsin accordance withHIPAA and company requirements.

• Partnerswithappropriate internalteams,when employee healthconcerns,intersect with broader workplace processes, whilemaintainingthe medical department's clinical responsibilities and scope.(ADD)

QUALIFICATIONREQUIREMENTS

Required:

• Bachelor’s degree inNursing(BSN)

• Current, unrestricted Registered Nurse (RN) license in the applicable state.

• Minimum ofthree(3)years experiencein direct clinical care to patientsrequired.

• Case Management, Medical Management, Field Case Management, Public Health Care, orworker’scompensation-related experience is a plus.

• Must be proficient in Microsoft Office suite with such programs as Word and Excel, as well as a comfort level with other software programs.

• Strong clinical assessment, critical thinking, communication, leadership, and organizational skills.

• Ability to proactivelyidentifyhealth concerns and developappropriate preventiveinterventions.

• Ability to work effectively with employees and leaders at all organizational levels.

• Strong computer, documentation, anddata-managementskills.

Preferred:

• Master’s degree in nursingwith valid state licensure.

• BLS/CPR certification.

• Spanish speaking ability highly preferred.

• Experience developing employee wellness, health promotion, and preventive healthprograms.

SCHEDULE
