Job summary

Azza HealthCare Agency in the United States seeks a registered nurse to join an interdisciplinary team, promoting preventive care, disease management, and patient education. You will provide primary clinical support, assist with triage, and develop individualized care plans.

Responsibilities include assessing needs, coordinating care, educating patients and families, implementing interventions, managing IVs, and supporting transitions of care within electronic health records.

Qualifications

• Graduate of a Specialty Training Program in Nursing.
• Bachelor’s Degree in Nursing preferred.
• At least two years RN experience, preferably in Med/Surg or hospital settings.

Responsibilities

• Assess healthcare, educational, and psychosocial needs of patients and families, involving them in treatment planning.
• Facilitate communication among patients, providers, and healthcare team members to eliminate barriers and coordinate interventions.
• Educate patients, families, and staff on disease management, clinical guidelines, and care coordination.
• Implement clinical interventions, monitor therapies, and adjust treatments as directed by providers.
• Coordinate diagnostic tests and maintain documentation within electronic health records.
• Participate in professional development, review current literature, and incorporate best practices into care.
• Support transitions of care, assist with laboratory procedures, medication administration, and IV management.
