Qualifications

• Minimum of 2 years LPN work experience.
• Graduate from Specialty Training Program-Nursing.

• Ability to work in a clinical environment and perform patient education.

Responsibilities

• Assist patient introduction to the program using clinical knowledge.
• Evaluate patient needs by phone or documentation and coordinate care plans.
• Communicate with patients via mail, phone, and in person with timely follow-up.
• Liaise between patients and providers, coordinating activities and support services.
• Develop and implement screening tools and navigational procedures.
• Review medical records, provide nursing care, and assist physicians.
• Maintain clinical supplies and monitor equipment condition.
• Address patient concerns related to appointments, communication, and billing.
