Job summary

The Center for Inherited Blood Disorders (CIBD) seeks an experienced RN Nurse Care Coordinator to manage patient care across the illness continuum. You will promote efficient resource use and lead a multidisciplinary team in a team-centered care model, complying with CCS, GHPP, and Joint Commission standards.

On-site role in California with a strong focus on coordination, collaboration, and program development within a supportive, mission-driven environment.

Qualifications

• Graduate from an accredited RN program and be licensed in California.
• Minimum of two years nursing experience in acute care or ambulatory settings.
• Ability to work onsite (not a remote role).

Responsibilities

• Coordinate patient care with internal and external providers to ensure seamless plans.
• Collaborate with physicians, nurses, and payers to fulfill orders from start to finish.
• Maintain strong documentation and effective written/verbal communication.
• Support the care team and participate in staffing cross-training when needed.
• Promote program development through outreach, committees, and events.
