Location: Shawnee Mission Area

The Connected Care Coordinator acts as the key patient advocate and educator for coordinating post-acute care services within AdventHealth-owned hospitals. This role involves assessing patients for post-acute care, coordinating clinical transitions to home health and hospice, and maintaining relationships with physicians, post-acute providers, therapists, patients, and families. The Coordinator will be assigned to a specific hospital or specialty and collaborates with care management, physicians, and clinicians to develop discharge plans involving post-acute services across AdventHealth's continuum of care.

The value you’ll bring to the team:

• Conduct systematic post-acute assessments of patients' physical, psychosocial, and functional status to determine appropriateness for home health and hospice care.
• Educate patients and families about post-acute settings, balancing their requests with safety and care quality considerations.
• Identify patient/family problems or needs and communicate these to the physician and clinical team.
• Assist with home health care referrals within assigned hospitals, including bedside assessments for admission appropriateness and educating families about discharge and care expectations.
• Support the intake process, including pre-registration requirements for home health care admission.

The expertise and experiences you’ll need to succeed:

Required:

• Associate degree in Nursing or higher
• At least two years of post-acute clinical experience or care management in a facility
• Experience working with the public and providing exceptional customer service
• Knowledge of medical terminology and post-acute referral processes

Preferred:

• Bachelor’s degree in a medical-related field

Licensure, certification, or registration required:

• Registered Nurse (RN)
• Active American Heart Association BLS certification
• Valid driver’s license, automobile insurance, safe driving record, and reliable transportation
