Job summary

KC CARE is seeking a Manager of Nurse Care Coordination (RN) to lead nursing programs, supervise Nurse Care Coordinators and staff, and ensure comprehensive, patient-centered care across medical, behavioral health, social, and community needs.

The role serves as the operational leader for population health nursing services, providing clinical oversight, workflow development, and quality monitoring in partnership with the VP of Population Health to meet regulatory and grant requirements.

Qualifications

• BSN in Nursing required and RN license in Missouri.
• 5 years in internal medicine/family medicine/community health.
• 2 years progressive management, including program planning, evaluation and budgeting.

Responsibilities

• Lead nursing programs and supervise Nurse Care Coordinators and nursing staff.
• Provide direct patient care as needed and ensure comprehensive patient-centered care.
• Ensure accurate documentation of patient care in the EHR.
• Oversee care management, transitions of care, and population health initiatives.
• Collaborate with VP of Population Health to set goals and quality initiatives.
