Job summary

The Terraces at Heritage Grove Estates in Topeka, Kansas is seeking a compassionate LPN or RN to join our Assisted Living team.

The Nurse provides nursing support and oversight for residents, monitors health changes, manages medications, and coordinates with families and providers to ensure safety and dignity. Opportunities exist to develop leadership skills in a supportive environment.

Qualifications

• Active Kansas LPN or RN license in good standing.
• Ability to communicate effectively with residents, families, and team members.
• Assisted living, senior living, long-term care, or related healthcare experience preferred.
• Experience working with older adults preferred.

Responsibilities

• Monitor health, safety, and overall wellness of residents.
• Complete nursing assessments and evaluations as assigned.
• Observe residents for changes in physical, cognitive, behavioral, or emotional condition.
• Obtain and monitor vital signs and other clinical information as appropriate.
• Provide nursing care and treatments within applicable scope of practice.
• Support residents in maintaining independence and safety.
• Respond to resident emergencies and urgent clinical concerns.
• Administer medications and monitor resident response.
• Coordinate with families and providers; review orders and documentation.
• Ensure continuity of care across care team and services.
