This position functions similarly to a traditional MDS Nurse / MDS Coordinator role and is responsible for coordinating resident assessments, supporting accurate clinical documentation, completing required MDS assessments, and helping ensure appropriate care planning and reimbursement.

If you are an RN or LPN with skilled nursing experience who understands the connection between great documentation, accurate assessments, quality outcomes, and reimbursement, we want to hear from you!

Position Overview

The Nurse Assessment Coordinator plays an important role in ensuring each resident's clinical needs are accurately assessed, documented, and reflected in their individualized plan of care.

Working closely with nursing leadership and the interdisciplinary team, this position coordinates the MDS assessment process, monitors documentation, supports care planning, and helps ensure compliance with federal and state requirements.

The ideal candidate is organized, clinically knowledgeable, comfortable working with deadlines, and able to collaborate effectively with nursing, therapy, social services, dietary, medical providers, and other members of the interdisciplinary team.

  • Participate in and support the development, review, and updating of individualized resident care plans.
  • Collaborate with the interdisciplinary team to ensure assessments and care plans accurately reflect each resident's current condition and needs.
  • Monitor nursing and interdisciplinary documentation for completeness, consistency, and accuracy.
  • Identify documentation opportunities and provide education to clinical team members as needed.
  • Support accurate PDPM classification and reimbursement through thorough assessment and documentation.
  • Review resident diagnoses, medications, treatments, therapy services, functional status, and other clinical information relevant to the assessment process.
  • Participate in care plan meetings and interdisciplinary clinical discussions.
  • Monitor significant changes in resident condition and ensure appropriate assessments are initiated when required.
  • Maintain compliance with MDS 3.0, RAI guidelines, CMS requirements, and applicable state and federal regulations.
  • Assist nursing leadership with quality improvement initiatives, audits, and survey preparation.
  • Monitor assessment-related quality measures and help identify opportunities for improved resident outcomes.
  • Maintain accurate, timely, and confidential resident records.
  • Serve as a clinical resource for facility staff regarding MDS, documentation, care planning, and assessment requirements.
