American Medical Administrators LTC in Kansas City, MO is seeking a Nurse Assessment Coordinator to oversee the assessment and care planning process for residents, ensuring MDS assessments and care plans are completed accurately and in line with federal and state guidelines.

You will collaborate with interdisciplinary teams, monitor data quality, participate in care plan meetings and audits, and educate staff on documentation best practices.

Job Responsibilities and Duties:

The Nurse Assessment Coordinator is responsible for overseeing the assessment and care planning process for residents in accordance with federal and state guidelines. This includes the coordination and completion of the Minimum Data Set (MDS) assessments, care plans, and related documentation to ensure accurate reimbursement and high-quality care.
