Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care in accordance with laws, standards, regulations, and Sunrise policies to ensure the highest quality of care.

Responsibilities & Qualifications

Responsibilities:

1. Support the Mission, Principles of Service, and Core Values, emphasizing the sacred value of human life.
2. Administer medications and treatments, ensuring safety and proper documentation.
3. Review, read, notate, and document pertinent resident information.
4. Receive medication updates from the RCD or Wellness Nurse.
5. Ensure medications are passed timely, documented, and residents’ confidentiality maintained.
6. Report resident concerns and assist with medication checks and restocking.
7. Maintain cleanliness and organization of medication areas.
8. Assess residents for 'as needed' medications and document accordingly.
9. Count narcotics with another MCM or LCM each shift.
10. Support resident independence and safety, practicing good hygiene and safety precautions.
11. Participate in resident care planning, updates, and team meetings.
12. Ensure compliance with safety, OSHA, infection control, and hazardous materials regulations.
13. Participate actively in team and community safety initiatives and meetings.
14. Maintain required certifications, training, and ongoing education.

Qualifications:

• Licensed nurse credential (LPN, LVN, or equivalent).
• Maintain certifications such as CPR and First Aid.
• Minimum age of 18 years.
• Experience working with seniors preferred.
• Strong communication skills, decision-making ability, and calmness in difficult situations.
