About the Role

We are seeking a compassionate, dependable LPN or RN to join our team in a full-time, on-site position. You will support residents in assisted living and memory care, working closely with caregivers, healthcare providers, and families to promote comfort, dignity, and well-being.

Responsibilities, consistent with your licensure and scope of practice, include:

• Monitoring resident health, recognizing changes in condition, and coordinating timely follow-up.
• Administering medications and treatments as ordered.
• Contributing to individualized care planning and ensuring care reflects residents’ needs and preferences.
• Maintaining accurate, timely documentation according to facility policies and applicable requirements.
• Providing direction, education, and support to care staff.
• Communicating clearly and compassionately with residents, families, and healthcare providers.
• Responding to resident concerns and emergencies with sound judgment.
• Supporting infection prevention, resident rights, and a positive, person-centered environment.

Qualifications

• Current LPN or RN license in good standing.
