Job summary

MJHS is seeking a Nurse Practitioner to staff palliative care consultations across nursing homes, hospitals, and community settings. The NP will assess symptoms, discuss goals of care with patients and families, and develop comprehensive care plans.

Travel within the assigned geographic area is required. A Master’s in Nursing and NP program graduation are required, with NY licensure and relevant certifications preferred.

Qualifications

• Master’s degree in Nursing and graduation from an approved NP program.
• New York State RN licensure and NP certification in relevant specialties.
• Experience with hospice, palliative care or geriatric nursing is preferred.

Responsibilities

• Staffs palliative care consultation services in nursing home, hospital or community settings.
• Develops care plans, conducts consultations, and follows up with patients and families.
• Performs history-taking, examinations, and documentation per best practices.

Education

Master’s degree in Nursing

Nurse Practitioner Program graduate

New York State RN licensure

NP certification: Adult/Family/Gerontology/Palliative Care
