Job summary

Adobe Population Health is seeking a Director to lead the Adobe Provider Group, overseeing NP, RN, Care Navigator, and LVN teams. You will drive program development, clinical quality, regulatory compliance, and cost-effective care aligned with mission and contracts.

You will manage daily field operations, monitor SLAs and KPIs, and design dashboards to track performance and outcomes. Strong leadership and NP licensure are required.

Qualifications

• Nursing leadership and population health program management experience.
• Experience hiring, training, and supervising multidisciplinary teams.
• Knowledge of SLAs, KPIs, Medi-Cal, and Medicare Advantage programs.

Responsibilities

• Lead field-based teams of NPs, RNs, Care Navigators and LVNs.
• Oversee program operations across medical, behavioral, and social domains.
• Ensure HIPAA compliance and data security; support audits and quality initiatives.
• Develop palliative care program for complex and end-of-life conditions.
• Collaborate with leadership to align goals and resources.
