Job summary

Adobe Population Health seeks a Director for the APG Program to lead field-based teams and oversee integrated care initiatives in Phoenix, AZ. This role drives program development, clinical quality, and regulatory compliance to fulfill contractual obligations.

The position requires NP- and RN-level leadership, population health experience, and strong collaboration with executives. Will travel in-state/out-of-state as needed and manage a diverse care team to deliver equitable outcomes.

Qualifications

• BSN required; MSN preferred.
• CCM and Palliative Care certifications within 2 years.
• Current BLS certification.
• Driver's license and valid insurance.
• Proficient with Salesforce and MS Office.

Responsibilities

• Lead field-based care teams including NPs, RNs, Care Navigators, and LVNs.
• Ensure SLAs and KPIs are met; monitor quality and outcomes.
• Develop and oversee a palliative care program for complex cases.
• Maintain HIPAA compliance and data security; support audits.
• Collaborate with senior leadership, payers, and providers.
