Job summary

SCAN, a nonprofit health organization serving seniors across the Northwest, is seeking an experienced nurse practitioner leader to manage primary care programs within an integrated care delivery model.

The role requires 5+ years NP experience, Oregon licensure, and supervisory skills; the team focuses on ethics, quality, and improving outcomes while expanding member services and collaborations. The position offers a base salary range and a comprehensive benefits package.

Qualifications

• Master’s degree from an accredited school of nursing.
• Oregon RN/NP licensure required; Furnishing licensure.
• Certification in area of specialty required.

Responsibilities

• Manage programs and clinicians providing primary care within the integrated care delivery model.
• Oversee clinical teams to achieve defined care outcomes.
• Ensure ethics and quality standards in patient care by Advanced Practice Providers (AAP).
• Select, on-board, train and develop clinicians within the integrated care delivery model.
• Develop and monitor success metrics; adjust programs to improve efficiency.
• Collaborate with senior management on operational and performance improvements.
• Promote membership growth and quality outcomes for integrated care delivery products.
• Maintain confidentiality and protect operations.
