Job summary

SCAN Health Plan in Oregon is seeking an experienced Nurse Practitioner leader to manage programs and clinicians delivering primary care within an integrated care delivery model. You will oversee performance, ensure high-quality patient care, and drive new plans and programs with senior management.

Role requires an Oregon RN/NP license, a master’s degree in nursing, and at least 5 years of NP experience, plus 3+ years of supervisory experience.

Qualifications

• Master’s degree in nursing required.
• Oregon RN, NP, and furnishing licensure required.
• Certification in area of specialty preferred.
• 5+ years NP experience required.
• 3+ years supervisory experience preferred.
• Experience with Medicaid and Medicare.

Responsibilities

• Manage programs and clinician teams delivering primary care within the integrated care model.
• Oversee daily operations, ensuring ethics and quality in patient care delivery.
• Select, onboard, train, and develop clinicians within the model.
• Develop and monitor care plans and documentation; educate patients and families.
