Job summary

Hackensack Meridian Health is seeking a Palliative Care Advanced Practice Nurse (APN) to lead and coordinate patient-centered care in a hospital setting in New Jersey. The role requires a NP with Master’s or Doctorate, NJ RN license, and prescriptive authority (CDS/DEA) with physician collaboration.

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office and Google Suite platforms.
• NJ State Professional Registered Nurse License.

• AHA Advanced Cardiac Life Support Certification.

Job description

The Palliative Care Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Minimum rate of $156,644.80 Annually
