Job summary

Hackensack Meridian Health's Transition Team Advanced Practice Nurse (APN) delivers patient-centered coordinated care, leading clinical practice and collaborating with the hospital team.

The APN participates in planning and implementing care, provides consultation, coaching, and research, and adheres to NJ licensing and prescriptive authority requirements. Weekend and holiday shifts are part of the schedule.

Qualifications

  • Graduate of an NLN/AACN accredited nursing program.
  • Master's or Doctorate from an accredited Nurse Practitioner program.
  • Maintains clinical competencies through continuing education and clinical practice.
  • National Provider Identifier (NPI).
  • Prescriptive authority through CDS and DEA registration according to New Jersey state law.
  • Collaborative agreement with physicians on staff.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include Microsoft Office and Google Suite platforms.

Responsibilities

  • Utilize a patient-centered coordinated care model to assess, plan, and implement care.
  • Lead and collaborate with the practice/hospital team on care delivery.
  • Provide leadership in direct clinical practice and consultations.
  • Coach and guide other staff and patients as needed.
  • Engage in research and uphold ethical decision-making in care plans.
  • Participate in weekend and holiday scheduling as required.

Job description

The Transition Team Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Weekend WorK: every Fourth Weekend

Holiday Work: Two of 6

Education, Knowledge, Skills and Abilities Required: 1. Graduate of an NLN/AACN accredited nursing program. 2. Master's or Doctorate degree from an accredited Nurse Practitioner program. 3. Maintains clinical competencies through continuing education and clinical practice. 4. Experience in a relevant patient care setting (or as regulations dictate). 5. National Provider Identifier (NPI). 6. Prescriptive authority through CDS and DEA registration according to New Jersey state law. 7. Collaborative agreement with physicians on staff. 8. Excellent written and verbal communication skills. 9. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Minimum rate of $156,644.80 Annually
