Job summary

Southcoast Health is seeking a highly skilled Advanced Practice Practitioner to join the Health at Home – Supportive Care Hospice – Palliative Consulting Team in a full-time capacity. The role involves collaborative practice with physicians to deliver hospice care and related services to patients and families.

Hours are 40 per week, 8:00 AM–4:30 PM, Monday–Friday, with no weekends, holidays, or on-call. The successful candidate will have an MSN, NP certification, an RN license in MA/RI, and

Qualifications

• Masters of Science Degree in Nursing.
• Certification from a national accrediting body to practice as a Nurse Practitioner is required.
• Current RN license in the Commonwealth of Massachusetts & Rhode Island is required.
• Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.
• A minimum of one-year Nurse Practitioner experience.
• Certification in Hospice and Palliative Care required. Must possess current CPR certification.
• Certification from a national accrediting body to practice as a Nurse Practitioner is required.
• Current RN license in the Commonwealth of Massachusetts is required.
• Certification in Hospice and Palliative Care required.

Responsibilities

• Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families.
• Perform a wide variety of duties including but not limited to bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc, and serving as a nursing resource for education and consultation to members of the interdisciplinary team.

Available to regular status employees who are scheduled to work a minimum of 24 hours.
