Job description

St. Luke's is seeking anAdvanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our highly collaborative Rheumatology team and support our growing Bone Health Program based in Meridian, Idaho. The selected candidate will join an established practice of 3 APPs and 3 physicians specializing in bone health, dedicated to delivering exceptional patient-centered care. Bone Health is a strategic, value-based initiative focused on improving the diagnosis, treatment, and long-term management of osteoporosis and other metabolic bone diseases. This innovative program partners closely with Orthopedic Surgery, Spine Care, Primary Care, Endocrinology, Women's Services, and Physical Therapy to provide comprehensive patient education and promote early intervention. Our goal is to improve both the quality and quantity of life for patients through proactive bone health management.

The Advanced Practice Provider will provide high-quality patient care through assessment, diagnosis, treatment, and ongoing management of patients with acute and chronic conditions. The APP will work under the supervision of a designated physician and collaborate closely with a multidisciplinary team of providers and support staff to ensure outstanding patient outcomes.

This role embraces team-based care and innovative care delivery models, utilizing virtual care and technology-enabled solutions to improve patient access, engagement, and continuity of care. The ideal candidate will be comfortable leveraging digital tools and working collaboratively across specialties to deliver efficient, coordinated care.

In addition to supporting the Bone Health Program, this position will spend up to three days per week embedded within the St. Luke's Spine Clinic, partnering closely with Spine providers to strengthen integration between Spine Health and Bone Health services and enhance the patient experience.   • Experience in Primary Care, Orthopedics, Spine Care, Endocrinology, Rheumatology, or other related specialties strongly preferred.
