Esse Health is seeking a compassionate Advanced Practice Provider (NP/PA) for our Phoenix Family Medicine office. This outpatient role focuses on adult primary care, with a collaborative team and physician partners to deliver high-quality care.

Qualifications

• Masters Degree of Science in Nursing or Physician Assistant-Masters Degree.
• Current Nurse Practitioner or Physician Assistant licensure in Missouri (or eligibility).
• Certified Physician Assistant or Certified Nurse Practitioner (or eligibility).
• Strong clinical, communication, and interpersonal skills.
• Commitment to patient-centered, team-based care.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Join a collaborative team and develop a 1:1 relationship with patients and clinical staff members.
• Provide comprehensive primary care services to adult patients.
• Conduct patient evaluations, physical exams, and diagnostic assessments.
• Diagnose and manage acute and chronic conditions.
• Develop and implement treatment plans in collaboration with physician partner.
• Emphasize preventive care, wellness, and patient education.
• Document patient care accurately and efficiently in the electronic health record (NextGen).
• Coordinate care with specialists and other members of the healthcare team.
• Participate in quality improvement and care coordination initiatives.
