HealthOp Solutions in Phoenix, AZ is seeking a Family Nurse Practitioner or Physician Assistant for adult primary care. The role offers a Monday–Friday outpatient schedule with a base salary of $120,000–$150,000 plus bonus potential and full benefits.

The ideal candidate may be new graduate or have 2–3 years of primary care experience, with licensure eligibility in Arizona and FNP/PA certification as applicable.

Qualifications

• Eligibility for NP or PA licensure in Arizona.
• Completion of an NP or PA program in the United States.
• FNP or PA certification, as applicable.
• Interest in adult primary care.
• New graduates welcome.

Responsibilities

• Provide outpatient primary care to adult patients.
• Perform patient evaluations, follow-ups, and wellness visits.
• Manage acute and chronic conditions.
• Order and interpret labs and diagnostic testing.
• Develop treatment plans and provide patient education.
• Prescribe and manage medications as appropriate.
• Collaborate with physicians, APPs, and clinical support staff.
• Maintain accurate and timely documentation.
• Support continuity of care in a team-based clinic setting.
