Qualifications

Responsibilities

• Assess, diagnose, and adjust treatment plans.

• Provide health promotion and disease prevention counseling.
• Collaborate with an interdisciplinary team and refer when needed.
• Document in the EMR and ensure compliant coding and billing.
• Support care coordination and communicate with health agencies.
• Perform procedures within granted privileges.
• Participate in quality, safety and peer-review initiatives.
• Educate and onboard new team members.
• Maintain productivity, access, and quality metrics for patient care.
